Toddler 1: For children 12 months by 9/1/2010. Toddlers may attend two days per week. * Please indicate your choice of days in the space provided.
* If you are registering for one the following classes, please indicate the number of days per week (2-5) and first and second choice of days, if fewer than 5. If you elect a three day per week schedule, you must choose Friday as one of your days.
Toddler 2: For children 24 months by 9/1/2010. Two-five days/week.
Preschool 1 (young 3s): For fully toilet-trained children 3 years old by 12/31/2010.
Preschool 2 (Three year olds): For fully toilet-trained children 3 years old by 9/1/2010.*

* Parents of young 4 year olds who do not wish to send their children five days per week may enroll them in the Preschool 2 class.

- Abide by the policies set forth in this application and all policies outlined in the Parents’ Handbook;
- Provide health and emergency contact information and my child’s current immunization records when requested;
- Participate as much as I am able in my child’s classroom and attend teacher/parent conferences as requested by my child’s teacher.
I understand that though this digital signature and by submitting this form, I am committing my child to KDO Preschool for the full school year. If I withdraw my child before the end of the current school year, I will be obligated to pay the full year’s tuition, unless the withdrawal is due to a move outside the area.
